We provide in home care to clients who qualify to receive these
services. We accept Medicaid, some private insurances, private pay, and workers
compensation insurance.
- Personal Care Services
Personal care services (PCS) are available for clients who have a medical
condition that requires direct and ongoing care by a physician, are medically
stable at home, and need help with personal care tasks due to the medical
condition. Personal Care Services are available to clients who don't qualify for
CAP services. PCS is to assist the help available from
family members and community resources. Children as well as adults are eligible
for this service.
Payment sources:
- Medicaid (60 -80 hours per month) If you have a "Blue"
Medicaid card
you may receive services with no direct cost to you.
- Private Insurance & Worker's Comp - Ask your
insurance company to see if you're covered
- Private Pay - Up to 24 hours per day 7 days per week
PCS provides an in-home aide for:
- Assisting with personal care, grooming, hygiene, shaving, toileting,
feeding
- Assist in the self-administration of medications
- Assisting with activities of daily living. (Ambulation, range of motion,
turning, reposition, transfers from bed-chair, personal hygiene, bowel and
bladder programs, etc.)
- Monitor vital signs, catheter care, non-sterile dressing changes,
prescribed heat and cold applications, and ensuring compliance with medical
routines.
- Assisting with errands, shopping, appointments
- Home management (cleaning, meal preparation)

Community Alternative Program

- A program funded by Medicaid that allows an individual
to remain at home. Services can include an in-home aide, respite care and
even nursing care for certain clients. The Community Alternatives Program has a
lead agency. There can only be one lead agency in each county. The lead agency is the starting place for CAP services. There will be case manager assigned to
your care through the lead agency. Each lead agency has a list of the agencies
that provide services in that county. As a client, you have the right to choose
which agency you want to provide your care. The lead agency cannot influence or
suggest a home care provider. You may choose any provider on the list. It does
not have to be the lead agency. A Medicaid funded program allowing an alternative to nursing
home placement.

Companions/Private
Sitters
-
- Our agency provides aides and nurses for companions and private sitters. This
service is available to anyone who needs it. Companions and sitters are provided
for shifts of at least 4 hours but can be provided for 24 hours a day. Services
must be for at least 20 hours a week. Companion services can be scheduled for
one week to an indefinite period of time.
Companions and sitters provide company for the client during an illness at
home or hospital/nursing home stay. They are employed through the agency so you
know that references and background checks have been done.

- Private Duty Nursing
Medicaid pays for Private Duty Nursing as long as it is a continuous, necessary service by a licensed
nurse that is needed in the client's home (ventilator dependent, device-based
respiratory support, tracheostomy care, and tracheal suctioning, etc.).
Besides
providing care in the home, the nurse may accompany a client outside the home
when the client's normal life activities take the client away from home (such as
a child attending school during the day).
Private duty nursing is available for children and adults.
Private duty nursing can also apply to a private pay or private insurance
situation.
Payment sources include: Medicaid, private insurance, private
pay, workers compensation.
Hourly
Nursing
- Hourly nursing can be used for many different reasons. These visits are
billed by the hour and usually are 1-2 hours a day 5-7 days per week.
Any of the following reasons may constitute a nursing visit:
- Vital sign monitoring
- Wound care (pressure ulcers, surgical wounds)
- Nasopharyngeal and tracheostomy aspiration
- Catheter Care (Irrigation, insertion or replacement of catheters, care of
newly inserted suprapubic catheters)
- Nasogastric, gastrostomy and jejunostomy tubes (placement, feedings, and
teaching)
- Insulin administration and teaching
- Oxygen administration set up, monitoring and teaching
- Diet teaching
Home Infusion Therapy

Home Infusion Therapy allows patients to
continue with their daily activities while receiving care in the comfort
of their home or alternate site setting. Generations works with the
prescribing physician who directs the overall management of the
patient’s care. We focus on managing each component of the therapy
through our comprehensive staff of clinicians and case managers. Our
commitment is to provide the highest quality of care, clinical
excellence, and customer service.
Clinical Expertise
Generations employs registered nurses who have extensive acute care backgrounds.
They evaluate patients for acceptance based on appropriateness for non-hospital
care. The nurses educate the patient and family members with respect to
medication and self-administration techniques. Education and assessment continue
during routine follow-up visits. Generations nurse clinicians are experienced in
infusion management and maintain and problem-solve the patient’s IV site and
catheter. Monitoring the patient on an on-going basis includes assessing
response to therapy, lab draws and coordinating other services the patient may
require. Generations works closely with certified nursing agencies as required
ensuring care standards are achieved.
Our pharmacy has extensive admixture experience
and are available as resources for drug information. They are active
participants in the patient‘s ongoing care plan. Our pharmacy has a complete formulary of drugs and
solutions. Infusion pumps and equipment are selected based on the latest
technology, cost-effectiveness, and most appropriate selection for each patient.
Medications and supplies are delivered to the patient’s setting in limited
quantities to control drug costs due to therapy changes.
Generations Home Health Agency continually strives to improve the quality, convenience and

We take great care to make sure you understand and can follow through with
your individualized course of treatment. Our infusion nurse consults with your
primary care physician to define a complete plan of care.
You will be educated on the equipment, medications and solutions,
self-monitoring techniques and proper management and disposal of materials. The
infusion nurse will also monitor your lab work, provide continuous support and
assess your progress.
Our number one goal is to provide you with the best care possible, making full
use of proven traditional methods and innovative new technologies. Our approach
combines professional infusion therapy nurses and clinical pharmacists with
special services including home delivery and lab monitoring.

Our IV nurses are specially trained and available to
patients 7 days a week, 24 hours a day.

Our nurses obtain and monitor lab work and give
specialized attention to specific and often complex multiple medical issues.

Our IV team keeps the physician informed of your
patient's condition on a regular basis.
Our service makes all the arrangements for home
infusion and nutrition support therapies, including the initiation and
supervision of a treatment plan, comprehensive patient and care-giver education,
close monitoring and the delivery of supplies and equipment.

to Generations Home Health Agency and we will: (1)
pre-certify and verify the patient's insurance coverage; (2) coordinate all
infusion needs; (3) coordinate all nursing care.
