When you fill out an advance directive form you are making sure that you will always get the care you want. It helps take the burden off family members who may have to make these decisions for you.

No one likes to talk about illness and death. It is much easier to do when you are healthy and relaxed. Your family needs to know your wishes. Illness and injury can happen anytime. With advance directives, your wishes will be respected.
